Pennsylvania Home Care One is seeking a dependable and organized Care Coordinator to join our growing home care agency. This is a hybrid position, combining office-based coordination with field visits for supervision and quality assurance.
The Care Coordinator will manage an assigned caseload of clients and caregivers, ensuring continuity of care, regulatory compliance, and high client satisfaction.
This role is ideal for someone with home care, healthcare administration, or staffing coordination experience who thrives in a fast-paced, mission-driven environment.

Requirements

  • Experience in home care, healthcare coordination, staffing, or related field preferred
  • Strong organizational and time management skills

  • Must complete required trainings and maintain compliance certifications

Compensation
  • $27,000 - 35,000 annually
  • Minimum 20 hours per week
  • Up to 40 hours per week based on agency needs

Responsibilities

  • Manage and maintain an assigned caseload of clients and caregivers
  • Conduct client intake assessments and assist with care planning
  • Coordinate caregiver schedules and assignments
  • Handle call-offs and emergency coverage needs
  • Maintain accurate documentation and client files
  • Ensure compliance with state regulations and agency policies
  • Monitor caregiver credentials, clearances, and required trainings
  • Assist with EVV monitoring and payroll verification
  • Conduct field visits for quality assurance and supervision
  • Maintain regular communication with clients, families, and caregivers
  • Support agency growth and operational efficiency
